Vanderbilt – Parent — Please print and complete the NICHQ Vanderbilt Assessment Scale – Parent version for all children and teens prior to initial session and as requested. Do not treat this as a group project; each adult should complete the Vanderbilt separately. Divergent results are actually useful, as children often behave differently with the various adults in their lives.
Vanderbilt – Teacher — Please request that a teacher — preferably one who knows the child well — complete the NICHQ Vanderbilt Assessment Scale – Teacher version.
